I acknowledge that Mountain Biking contains certain risks and dangers to which I expose myself, and that a great number of these may not be indicated. The risk of injury from the activities involved in this activity is significant, including the potential for permanent paralysis and death, and while particular skills, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist
I KNOWINGLY ACCEPT AND ENTIRELY ASSUME THE SAID RISK AS WELL AS THE POSSIBILITY OF BODILY INJURY, DEATH, DAMAGES, AND LOSS OR PROPERTY WHICH COULD ENSUE, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES
I willingly agree to comply with the stated and customary rules, terms and conditions for participation in this activity. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the organizers immediately
On my behalf and on behalf of my heirs, immediate family, executors, successors, administrators or other representatives, I hereby WAIVE MY RIGHTS to any present and future claim made against Club Mont Ste Marie, its officers, directors, employees, volunteers, agents, the owners of the premises of the activity and their insurers (hereinafter jointly named “the releasees”) and further agree to hold the releasees exempt of all responsibility resulting from any and all loss, damage, bodily harm, or expenses subject to occur, during use of installations or due to my presence in the vicinity of these for whatever reason.
I hereby agree to indemnify and to hold harmless the releasees of all responsibility resulting from damages to goods or injuries caused to a third party following the use on my part of the installations, or due to my presence in the vicinity of these.
I ATTEST HAVING READ AND UNDERSTOOD THE CONDITIONS OF THE PRESENT CONTRACT AND KNOW THAT BY SIGNING I RENOUNCE TO EXERCISE SOME RIGHTS THAT MIGHT BE EVOKED AGAINST THE RELEASEES.

This is to certify that I, as parent/guardian with legal responsibility for this participant, do consent and agree to the terms of this waiver and release on behalf of the minor participant.
